2009 Enhanced Tribal Lifeline Application


LIFELINE & LINK UP SERVICES

The Lifeline Telephone Assistance Program and Link-Up assistance make residential telephone service more affordable by offering discounts on monthly recurring telephone charges and installation charges for qualifying residential low-income customers.

In order to be eligible for Lifeline and Link-Up assistance, the telephone number must be the subscriber’s primary residence, and the household income must be at or below 150% of the poverty level. 

An applicant cannot be a dependent under sixty (60) years of age on someone else’s Federal Income Tax return, and his/her total gross monthly household income must be at or below the levels shown in the table below:

          Michigan Family Independence Agency clients may qualify.

If you qualify for the Lifeline program, a discount will be applied to your monthly telephone bill.  Link-Up assistance offers qualifying subscribers up to a 50% reduction on new telephone installation service charges.  Subscribers may be subject to an annual verification process to ensure their continued Lifeline eligibility
